Once you have paid the non-refundable retainer fee, you will be registered for one of our upcoming classes. If the deposit is not made with the form, then the registration is incomplete. 

•   Fill out all of the highlighted areas on the form! 
• Make sure to fill out the "Classroom Course Dates" when registering for a class. 
• Please make sure to have at least two working contact numbers.  

**We ask that you keep in consideration that the class you are requesting may be full,

 so please call the office or email us to make sure the class is still available. **

Please proofread all the information before submitting the form. Once you have submitted the form and payment, we will send a confirmation to the email provided. If you do not receive a confirmation email within 24-48 hours, you must call us to confirm your submission.
